Every other year, schools which receive federal Safe and Drug Free funds are required to administer the California Healthy Kids Survey (CHKS) to students in grades 5,7,9, and 11. The CHKS measures student experience with and attitudes toward various risk behaviors and provides local schools and communities with data to identify the needs of their youth and to guide efforts to meet those needs. Evergreen fifth and seventh grade students will take this survey the week of November 16. Your student should have brought home a permission slip for participation in this survey. Parents of fifth grade students are required to return the permission slips whether or not they give permission for their child to participate while parents of seventh grade students only return the permission slip if they choose to not have their child participate. Please note that the survey is available for review in the school office or by clicking on one of the links below. Should you have any questions, please contact your school’s principal.
